The right eye bag treatment depends on which of four problems sits under your eye. Puffiness from fluid answers to cold, sleep and less salt in the evening, and fine lines over it answer to radiofrequency. A hollow that casts a shadow needs volume, and volume means filler injected by a doctor. Brown pigment in thin skin fades over months with sunscreen and gentle brightening. A fat pad that has pushed forward needs lower eyelid surgery: creams, light and machines leave fat where it sits.

Find your type before you compare prices
Two checks at home sort most faces. Take one photo as you wake and another after work, around six in the evening. Swelling that has gone down by evening is fluid; a lump that matches in both pictures is fat. Next, switch off the bathroom light and shine your phone's torch up at your face from below. If the dark band vanishes, a groove was casting it. If it remains, the colour lives in the skin or in the vessels beneath it.

Eye cream: hours of change
Cleveland Clinic doctors write that a cream with caffeine or retinol may make eye bags look smaller for a while. Cold does part of the same job for free, and so does a higher pillow.
The strongest trial of an eye cream for dark circles and puffiness that we found followed 40 women aged 35 to 60 for 12 weeks, and 37 finished. Instrument readings showed less pigment and less congestion in the small vessels under the eye. The cream's maker ran the study, and it had no comparison group, so it cannot separate the cream from 12 weeks of better habits. Cleveland Clinic warns against hemorrhoid cream under the eyes. Hydrocortisone, the steroid in many of those creams, makes skin thinner over months of use and may discolour the patch you wanted to lighten. Radiofrequency: heat that tightens skin
A radiofrequency device sends current through the skin, and the skin's resistance turns it into heat in the dermis, the layer under the surface. The heat tightens collagen on the day, and the skin keeps tightening as it heals over the following months.
The largest early study treated 86 people once around the eyes with a high-energy device in doctors' hands. Six months later, graders looking at blinded photos found wrinkle scores better by at least one point in 83% of treated areas. The treating doctors, looking without the before photos, saw an improvement in 29%. Half the patients said they were satisfied. Burns occurred in 21 of 5,858 applications, and three people kept small scars.
Read those numbers as a ceiling. A strong session moved fine lines one step for most people, and the change was small enough that doctors missed it without the photos. Radiofrequency firms skin. It does not take out fat or add volume to a hollow.
Red LED: skin quality over many sessions
Red light heats nothing. In a 2014 controlled trial, 113 volunteers had red or broad-spectrum light twice a week for 30 sessions, about 15 weeks, and 23 had none. The treated groups ended with smoother skin on surface scans and denser collagen on ultrasound. Red light alone did as well as the broader spectrum.
That trial treated face and body skin and did not measure eye bags. Read it as evidence for thin, crepey skin under the eye. It says nothing about fluid, fat or a hollow.
Filler: the option for a hollow
If the torch test made your dark circle vanish, you are looking at a groove, the tear trough, and the fix is volume. A doctor injects hyaluronic acid gel along it.
A 2024 review of 31 reports covering 2,556 patients found 91% satisfied. Swelling followed in 19% and bruising in 18%. Lumps showed in 5%, and a blue tint under the skin, called the Tyndall effect, in under 1%.
Some problems arrive late. A 2022 review of 52 delayed cases found swelling most often, with problems starting an average of 16.8 months after the injection, and hyaluronic acid caused most of the swelling. If your eyes already puff in the morning, raise that with the doctor before you book. Choose the injector for experience in the tear trough.
Surgery: the option for a fat pad
A lump that matches in your morning and evening photos is fat. As the tissue around the eye weakens with age, fat slides forward into the lower lid. A lower blepharoplasty changes it: the surgeon opens the lid through a line just below the lashes or from its inner surface, takes out or moves the fat, sometimes trims skin, and closes with fine stitches.
Belo Medical Group gives 10 to 14 days before you return to normal activity. Milan Eye Center tells patients to avoid strenuous exercise and swimming for about three weeks. Both quote results lasting five years or more. It is the one option on this list that changes a fat pad.
Pigment needs a slower plan
None of the five clears brown pigment under the eye in a few sessions. Sunscreen up to the lash line and sunglasses stop it darkening. Brightening has to stay gentle: under-eye skin is thinner than any other skin on your face, and each irritation adds pigment. Rule out a medical cause first
Get a doctor's opinion before any cosmetic treatment if the swelling stays past a day or two (Cleveland Clinic puts the line at 24 to 48 hours), if it is painful, itchy or red, or if it arrives with swollen ankles, a staring look or double vision. Allergy, a kidney condition such as nephrotic syndrome and thyroid eye disease can all show up first as puffiness. The eye bags guide lists each warning sign.
