Caffeine, vitamin K, vitamin C, retinol, niacinamide: what each did in a trial, how long it took, and which eye bags no cream reaches. By a Makati therapist.
An eye cream can add water to thin skin, soften fine lines over two to six months, and lighten blue or purple darkness a little. For brown circles, niacinamide and vitamin C take months and need daily sunscreen alongside them. No eye cream removes a fat pad or fills a hollow, and those two cause the bags that look the same from morning to night. Choose a cream by its active ingredient, matched to the problem you have, and ignore the promise on the box.
The evidence is thin, and you should know how thin. A 2024 review of eye cream ingredients found few trials of an eye cream on eye skin. Researchers tested most of these ingredients on the face or the arms. In the United States, eye creams sell as cosmetics, and cosmetics need no trial to prove they work. Each study below is small, so we give its size and length and let you weigh it.

Caffeine: puffiness and blue circles, one small trial
Caffeine is the ingredient most puffy eyes creams lead with. In an Iranian trial, 11 women wore a pad with 3% caffeine and 1% vitamin K under one eye and a water pad under the other for four weeks. The treated side showed shallower wrinkles and lighter dark circles. Eleven women, four weeks, a pad in place of a cream and water as the comparison: treat the result as a hint. The 2024 review rates caffeine safe on skin up to 3%.
For morning puffiness, add cold: Cleveland Clinic notes that cold shrinks dilated vessels. Keep the tube in the fridge, or use a cold compress for eye bags before the cream goes on. No cream changes the salty dinner or the short night that put the fluid there.
Vitamin K: the best result on blue circles, none on brown
In a Japanese study of 57 adults, a gel with 2% vitamin K (listed as phytonadione), 0.1% retinol and 0.1% vitamins C and E went on twice a day for eight weeks. Pooled blood under the eye decreased in 27 of them, 47%. Wrinkles eased a little. Brown pigment showed no clear change. The study had no comparison group using a plain gel, so some of that 47% may come from time and attention. The pattern still helps you: vitamin K belongs in a cream for the blue and purple type.
Vitamin C: the form on the label decides it
The case for vitamin C is thicker skin over the vessels. In a split-face study of 14 people, 10% sodium ascorbate went under one eye for six months and a plain lotion under the other. The vitamin C side reddened less, and its skin trended thicker without reaching significance. A second form, ascorbic acid glucoside, did no better than the plain lotion. A cosmetics company's laboratory ran the study. Read the ingredient list for the form and give it months.
Retinol: fine lines, with care
Retinol has the strongest evidence for fine lines, though none of it comes from eyelids. In a study of 36 adults with a mean age of 87, 0.4% retinol went on one arm up to three times a week for 24 weeks. Fine wrinkling improved and biopsies showed more collagen being made. The 2024 review lists redness, peeling and dryness as the common complaints, and the skin under your eye can be as thin as 0.2 mm. The arm study used it three times a week at most, so start there, keep it off the lash line, and wear sunscreen the next morning.
Niacinamide: for brown circles, under sunscreen
In a trial of 202 women aged 40 to 60, a moisturiser with 4% niacinamide and 2% N-acetyl glucosamine, worn with SPF 15, reduced brown spots more than SPF 15 alone over ten weeks. That was facial skin. Under the eye, much brown pigment sits deeper: in 200 patients in India, a Wood's lamp placed it in the dermis in 60.5%, and DermNet writes that creams do little for pigment at that depth. Use niacinamide for the part it can reach, and keep sunscreen and sunglasses as the base.
Hydroquinone and tretinoin also fade pigment, and both irritate skin this thin. A Korean review lists both and warns about irritation near the eye. Use them under a dermatologist.
Hyaluronic acid, peptides and ceramides: water and texture
The 2024 review collects the evidence. A low-molecular-weight hyaluronic acid cream, used twice a day for 60 days by 76 women aged 30 to 60, improved hydration, elasticity and roughness around the eyes against a placebo. A moisturiser with the peptide palmitoyl-KTTKS reduced fine lines over 12 weeks in a split-face trial. A ceramide cream raised the water content of eyelid skin. These three change texture and lines. None of them changes colour.
Leave out fragrance
Eyelid skin reacts to products the rest of your face tolerates. In North American patch-test data on 50,795 patients, 2,332 had dermatitis on the eyelids alone, and 43.4% of those had an allergy to something that touched the skin. Fragrance mix and the preservative methylisothiazolinone each turned out relevant in 16.5% of them. Pick a fragrance-free cream, add one new product at a time, and stop if your lids turn red, itchy or scaly. A dermatologist can patch test to find the cause.
No trial has compared an eye cream with a face cream carrying the same actives. Eye creams tend to be thicker, with more oil, which suits dry lids. If your face moisturiser is fragrance-free and carries the ingredient you need, it will do the same job.
When a cream is the wrong tool
Look at your under-eye at 7am and at 6pm. Our eye bags guide explains how to tell a fat pad from fluid. A bulge identical in both photos is fat, which no cream reaches, and a hollow needs a doctor with filler. Of the four kinds of under-eye bags, fat pads pushed forward are the one a surgeon handles.
See a doctor before buying anything if your lids swell along with your ankles or legs, or your urine turns foamy: NIDDK lists these as signs of nephrotic syndrome, a kidney condition. Thyroid disease and hay fever also swell the lids, per DermNet, and Cleveland Clinic treats swelling under one eye as a reason to call a doctor.

Can I use retinol under my eyes?
Yes, with care. The skin there can be 0.2 mm thin, and redness, peeling and dryness are the common complaints. Start two or three nights a week and wear sunscreen in the morning.
Do I need a separate eye cream?
No trial has compared an eye cream with a face cream of the same ingredients. A fragrance-free face moisturiser with the active you need does the same job.
