A cold compress shrinks eye bags made of fluid. Wrap a cold pack, crushed ice or a bag of frozen peas in a clean cloth, lie back, and rest it on your closed eyes for up to 15 minutes. Stop at 20 minutes, and keep ice off bare skin. The puffiness goes down for the morning and returns when its cause does. Ice cannot shift a bulge of fat, fill a hollow or lighten brown pigment under the eye, because cold works on swelling and those three hold no extra fluid.

How to use a cold compress on eye bags
1. Pick your cold. Any of these works. Wrap everything except the washcloth.

2. Lie down and close your eyes. Lying back keeps the compress in place without pressing. The American Academy of Ophthalmology advises very gentle pressure. The skin under your eye is thin, so let the weight of the compress do the work.
3. Time it. Up to 15 minutes, the figure Medical News Today takes from the US National Eye Institute. A few minutes of a cool washcloth also helps, and the AAO and Cleveland Clinic both describe that shorter version. Never go past 20 minutes with ice, to avoid frostbite.
4. Repeat if you need to. Medical News Today allows a repeat every couple of hours.
Do it in the morning, when fluid bags are at their largest after a night lying flat.
What the cold does to a puffy eye
Cold narrows the small blood vessels in your skin. Less fluid seeps out of them into the loose tissue under the eye, and the swelling stops building while the fluid already there drains. When the skin warms again, the vessels widen and the seepage resumes at the rate your salt, sleep and allergies set.
The trials on cold and eye swelling all measure swelling after surgery, where doctors can score it day by day. None of them tests everyday morning bags, and their results disagree.
- Cold helped. In a trial of 97 patients in Korea after brain surgery, cold gel packs went on the area around the eyes for 20 minutes every hour for three days. The average eyelid swelling score came to 0.59 in the cold group and 2.29 without cold. Bruising appeared in 23% of the cold group and 53% of the others.
- Cold made no difference to swelling. In a trial of 38 patients after upper eyelid surgery, surgeons iced one eyelid and left the other. Swelling, redness and bruising matched on both sides at one hour, one day, one week and two months. Pain on day one came out a little lower on the iced side, a median of 0 against 0.5 out of 10.
- Cold gave short relief.A 2025 review of 29 trials and 2,287 patients having eyelid surgery found that cold compresses gave short-term relief, and that magnesium sulfate dressings brought swelling down more than ice packs did.
For a puffy morning, expect the cold to soothe and to shrink the bag for the short term. The surgery trials say nothing about how long the effect lasts on an ordinary morning bag.
Can ice remove eye bags?
Ice removes one kind, for a while. Cold acts on each kind of eye bag in a different way.
Fluid puffiness. Ice shrinks it for the morning. It returns the next morning if the cause stays: a flat pillow, a salty dinner, an allergy or short sleep. Our guide on how to get rid of eye bags covers the habits that keep fluid off and how long each one lasts.
A fat pad pushed forward. No. The bulge is fat that has slipped past the membrane holding it behind the eye. Cold does not move fat, and the bulge looks the same morning and night. A surgeon removes or repositions it.
A hollow. No. The dark band is a shadow cast by lost volume, and cold adds no volume. A doctor can fill it.
Brown pigment. No. The colour sits in the skin cells, and cold does not change how much pigment they hold.
Ice cubes rubbed straight on the skin, a trend on TikTok and Instagram, break the first rule of the compress: Medical News Today says never to put bare ice on the skin. Wrap the cube in a cloth and you have a compress again.
Keep ice off bare skin, and watch for cold hives
Bare ice can cause a cold burn on skin this thin, and an iced compress left on for more than 20 minutes risks frostbite.
A smaller group of people react to cold itself. Cold urticaria raises itchy welts and swelling where cold touches the skin. In a report on three eyelid surgery patients, two swelled more after routine cool compresses, and the cause turned out to be cold urticaria. The authors warn that in rare cases the condition turns dangerous. If your eyes itch, welt or swell more after a compress, stop using cold and tell a doctor.
When puffy eyes need a doctor
Cold can hide swelling that a doctor should look at. Skip the ice and book a doctor when:
- puffiness is still there a day or two later. Cleveland Clinic's ophthalmologist draws the line at 24 to 48 hours, and the AAO sends you to an ophthalmologist for pain, itch or redness around the eye;
- your feet and ankles puff up at the same time as your eyes, or you see foam in the toilet after you urinate. MedlinePlus groups those symptoms under nephrotic syndrome, which affects the kidneys;
- your eyeballs look pushed forward or you see two of everything. MedlinePlus ties those changes to Graves disease, an overactive thyroid;
- itching and sneezing arrive with the puffiness on most days. That pattern suggests an allergy, and a doctor can test for the trigger.

A compress handles one morning. Once the swelling is down, thin and creased skin under the eye stays as it was, and cold does nothing for it.
