Loose jawline, crepey cheeks or a soft neck? A Makati therapist explains which skin tightening treatment fits which problem, and when only surgery works.
A jawline that lost its edge and cheeks that feel thin and crepey look like one problem in the mirror. Under the skin they are two, sitting at different depths, and a clinic treats them with different machines.
Buy the wrong one and you pay for a course of sessions that improves the part of your face you were not worried about. This guide shows you how to tell which problem you have and which treatment reaches it.
Find the layer first
Your face has three layers that sag in different ways.
The skin. Collagen in the dermis thins with age and sun. The surface turns crepey, fine lines multiply, and the skin wrinkles like paper when you pinch it. The shape of your face has not changed yet.
The SMAS. Under the fat sits a sheet of connective tissue called the superficial musculo-aponeurotic system. When it loosens, the shape changes: the jawline softens, jowls appear, the cheek sits lower than it did in photos from five years ago. A surgeon lifts this sheet during a facelift.
Excess skin. After a large weight loss or decades of laxity, there is more skin than face. It folds and hangs.
A two-minute test in front of a mirror tells you where you stand.
Lie on your back with a hand mirror above your face. Gravity pulls the tissue back toward your ears. If your jawline looks five years younger lying down, the deep layer has dropped. If your face looks the same lying down but the skin still looks crepey, the problem sits in the skin.
Then pinch the skin on your lower cheek. If it snaps back at once, the skin is in good condition. If it stays tented for a second or two, it has lost elasticity.

Your jawline has softened: HIFU
HIFU stands for high-intensity focused ultrasound. The handpiece passes through the surface without heating it and focuses energy at a fixed depth, the way a magnifying glass gathers sunlight at one point.
At 4.5 millimetres it reaches the SMAS, the same layer a surgeon tightens. At 3.0 millimetres it works in the deep dermis. At 1.5 millimetres it treats thin skin around the eyes. The heat leaves small points of injury, and your body lays down new collagen along them over the following weeks.
Choose HIFU when the lying-down test changed your jawline, when jowls have started, or when the brow sits lower than it used to.
What to expect. Little change on the day. The lift shows between the second and third month and lasts about a year. Most clients rate the discomfort at 4 to 6 out of 10 on the deep passes along the jaw, lower on the cheeks. You can go back to work the same afternoon.
Where it fails. HIFU cannot remove excess skin. If the skin folds, the ultrasound has nothing to tighten it against.

Your skin looks thin and crepey: radiofrequency
Radiofrequency heats a broad area of the dermis. Collagen contracts when it warms, and the skin rebuilds collagen over eight to twelve weeks.
RF works shallower than HIFU and spreads its heat wider. HIFU lifts. RF firms.
Choose RF when the lying-down test changed nothing but the skin looks thin, crepey or dull, when fine lines cover the cheeks, or when the neck skin has softened without a visible drop.
What to expect. The session feels like a hot stone moving over the face. Your skin looks pink for about an hour and you can wear makeup the same day. The skin can look tighter in the first days, partly from mild swelling, and the real change arrives after two months.
A name to check. Thermage is a trademarked monopolar RF device from one company, priced at the top of the market. Many clinics in Manila run other RF systems with similar names. Ours is called Thermagical RF, and it is a different device. Ask any clinic for the machine name before you compare prices. Loose skin and acne scars together: RF microneedling
RF microneedling sends radiofrequency through fine needles into the dermis. It treats firmness and texture together, and it reaches acne scarring that surface RF cannot touch.
You pay for that with downtime. Expect redness and small marks for three to five days, and a course of three sessions spaced four weeks apart.
Choose it when you have loose skin and pitted acne scars at the same time. If you only have laxity, surface RF or HIFU gives a similar firming effect without the recovery days.
Can a firming cream do it?
A firming cream hydrates the surface and makes crepey skin look smoother for a few hours. It does not reach the SMAS, and no topical product rebuilds collagen at the depth where a jawline drops. We wrote a longer piece on why skincare alone cannot restore facial firmness.
Keep your retinoid and your sunscreen. Sun exposure breaks down collagen faster than any treatment builds it, and in Metro Manila that exposure runs all year. The American Academy of Dermatology recommends daily broad-spectrum sunscreen of SPF 30 or higher on all uncovered skin. Spend on skincare to protect the result, and spend on a device to create it.
When surgery is the honest answer
Three signs mean a device will disappoint you:
- skin that folds or hangs when you sit upright
- deep jowls that stay visible when you lie down
- loose neck skin that forms bands or a wattle
A facelift or neck lift removes skin. HIFU and RF tighten what is there. If you have these signs, see a board-certified plastic surgeon. A clinic that sells you a HIFU course for hanging skin is selling you a year of waiting.
Loose skin on the stomach and arms
The same logic applies to the body. Mild laxity after pregnancy or weight loss responds to RF and body contouring devices. A stomach with an apron of skin needs an abdominoplasty.

You have both problems: combining HIFU and RF
Some faces have both problems: a softened jawline and crepey cheek skin. HIFU treats the deep layer and RF treats the surface, so the two do not overlap.
Space them. We plan HIFU first, then RF four to six weeks later, once the swelling from the deep treatment has settled. Both results build over the same three months.
How long results last
Both treatments work by rebuilding your own collagen, and your collagen keeps ageing. HIFU results hold for about a year. RF firmness from a course holds six to twelve months. Most clients book a maintenance session once a year.
Surgery lasts longer, often eight to ten years, and costs more, with a recovery measured in weeks.
