Two women book HIFU in the same week. One walks out at three months looking noticeably lifted along the jaw. The other sees close to nothing and feels she wasted her money.
Neither therapist did anything different. The difference was decided before either of them sat down, and it is the thing most clinics will not tell you before taking your booking.
What HIFU does
HIFU stands for high-intensity focused ultrasound.
Ultrasound passes through the surface of your skin without affecting it, and focuses its energy at a set depth underneath. At the focal point the tissue reaches a temperature high enough to contract the collagen already there, and to signal your body to build more.
The deepest setting reaches the layer surgeons tighten during a facelift. That is why HIFU gets called a non-surgical facelift, and it is also where the marketing runs ahead of the truth. Surgery cuts and removes. HIFU tightens what you have.
The lifting effect comes from two things happening on different timelines. The contraction is immediate and small. The new collagen takes months and does the real work.
Who it works on
HIFU rewards mild to moderate laxity. If you can see your jawline softening and your cheeks sitting lower than they did five years ago, you are the right candidate. Most of our clients here are between 35 and 55.
It also works on the neck, under the chin, and around the brow.
Who it will not satisfy: anyone with heavy, hanging skin. Once the skin has lost that much support, ultrasound has nothing left to tighten and the result disappoints. A plastic surgeon gives that patient a real answer. We would rather tell you that at the assessment than take your money.
Weight also matters. Deep fat under the chin will not respond to tightening.
What to expect and when
This is where most disappointment comes from, so read this part twice.
Day one: a slight tightening, sometimes mild swelling. Do not judge anything by this.
Weeks two to six: little visible change. Many clients decide it failed during this window. Collagen is being built and nothing shows yet.
Month three: the result. This is when you compare photographs and see the jaw and cheek line.
Twelve to eighteen months: the effect holds, then fades as your skin continues to age. Most clients repeat once a year.
Take a photograph before your session, in the same light and the same angle you will use later. Memory is a poor judge of a gradual change, and without the photograph you will underrate your own result.
What it feels like
You will feel deep heat and small prickling sensations that follow the applicator along the bone. Along the jawline and brow, where the skin sits close to bone, it is sharper.
The sensation lasts only while the applicator is moving. Most clients rate it 4 to 6 out of 10 and get through it without numbing cream, though we offer it.
A full face and neck session runs 60 minutes.
Afterwards you may have mild redness for a few hours and some tenderness to touch for a couple of days. You can go back to work the same day.
About the number in front of HIFU
You will see clinics advertising 3D, 7D and 12D HIFU, and you will see the number used as though a higher one is better.
The number refers to how the device delivers energy across depths. It describes the machine, not your result. What decides your result is the depth chosen for your face, the amount of energy delivered, how many lines the therapist places, and whether the practitioner knows where the nerves run.
Ask how many shots are included in the price. Two clinics can both sell you "HIFU" and deliver a threefold difference in energy. That question tells you more than any number in the brochure.
HIFU or a thread lift
Clients ask us to compare these, so here it is plainly.
Threads place dissolvable sutures under the skin and pull tissue into position. The lift is immediate and mechanical. It suits someone who needs repositioning rather than tightening. It is a minor procedure with bruising and a recovery period, and it is performed by a doctor.
HIFU tightens the tissue where it sits. Nothing is inserted. The result is gradual and there is no recovery period.
Someone with sagging that needs to be moved upward is a thread patient. Someone with skin that has loosened but is still in place is a HIFU patient. Some people are told they need both, and that advice usually comes from a doctor rather than from us.
Who should wait
Do not book if you are pregnant or breastfeeding, if you have metal implants or fillers in the treatment area without discussing it first, if you have an active skin infection or open wound in the area, if you have a pacemaker or implanted electrical device, or if you form keloid scars.
If you have had fillers, tell us what, where, and when. Ultrasound energy can affect some fillers and we plan around them.

Is HIFU safe on Filipino skin?
