Pekas in English is freckles, but half of what Filipinos call pekas is not. What each type is, what fades it, and what to stop buying. ICONIQUE Makati.
Pekas in English is freckles. That is the answer most people came here for, and it is only half true, because half of what gets called pekas is not freckles at all. Sorting out which one you have decides whether anything you buy will work.
The three things Filipinos call pekas
Freckles. Small flat tan dots, mostly across the nose and cheeks, darker after sun and lighter in the rainy months. They run in families and they appear early, often in childhood. Nothing has gone wrong with your skin.
Sun spots. Solar lentigines. Larger than freckles, with a defined border, and they do not fade in the wet season. These are cumulative sun, and they turn up from the late twenties onward on the face, the forearms and the tops of the hands.
Melasma. Broad soft-edged patches on the cheekbones, upper lip or forehead, usually on both sides of the face. This one is not pekas at all, and it is the one people waste the most money on, because every cream aimed at freckles fails against it. If your marks are patches rather than dots, read melasma instead.
A quick way to sort it yourself: dots that darken every summer and fade every rainy season are freckles. Dots that stay put are sun spots. Patches that come and go with pregnancy, the pill, or a hot month are melasma.
What actually removes pekas
Sunscreen first, and this is not the boring answer
Ultraviolet light darkens freckles and drives the development of sun spots, though with freckles an inherited tendency does most of the work. Without consistent broad spectrum sunscreen, freckles darken again and new sun spots keep arriving under year-round Manila sun. It is also the cheapest thing on the list.
What you can buy
Ingredients with evidence behind them: niacinamide, azelaic acid, vitamin C, alpha arbutin, tranexamic acid, and retinoids for turnover. They lighten epidermal pigment slowly. Twelve weeks is a fair trial. Four days is not.
What to stop buying: any cream that promises whitening in a week, and anything of unknown origin sold through a chat app. Unregulated lightening products have repeatedly been found to contain mercury or potent corticosteroids that the label does not declare. Both lighten quickly, both damage skin, and stopping a hidden steroid can leave the marks darker than they started.
Gamot sa pekas that works is boring and slow.
What a clinic can do
For freckles and sun spots, which sit in the upper layers, pigment responds well to controlled energy.
A picosecond laser delivers its energy in pulses short enough to shatter pigment without pushing much heat into the surrounding skin. That restraint matters on Filipina skin: too much heat on Fitzpatrick IV to V can leave post-inflammatory darkening, the opposite of what you came in for. Spots typically darken for a few days after a session, then flake away.
A carbon laser facial is the gentler cousin, aimed at overall tone, pores and dullness rather than at individual spots.
Diamond microdermabrasion and peels work on texture and the most superficial pigment. For defined spots they play a supporting role.
For genuine childhood freckles, a clinic can lighten them, and they will come back with sun exposure, because the tendency is genetic. Anyone selling you permanent freckle removal is describing something they cannot deliver.
What a course looks like
Two to four sessions for scattered sun spots, three to four weeks apart, with sunscreen throughout. Freckles need maintenance rather than a course. Melasma is a different plan, longer and more conservative, covered in the melasma article.
Depending on the spot and the settings used, treated pigment often darkens for a few days or forms a light crust before it flakes away. Blistering, pain that builds rather than fades, or anything you did not expect should be reported to the clinic rather than waited out.
When to see a doctor instead
Any single spot that is growing, changing shape, sitting asymmetrically, showing more than one colour, or bleeding belongs with a dermatologist rather than in a facial chair, whatever your age and however small it is. A mark left after acne or an insect bite is usually post-inflammatory pigmentation. Many of those fade on their own, and the ones that do not are worth having looked at before you treat them as pekas.
Frequently Asked Questions
Ano ang pekas in English?
Freckles. In practice, Filipinos use pekas for freckles, sun spots and sometimes melasma, which are three different things with three different treatments.
Ano ang mabisang gamot sa pekas?
There is no single one. Daily SPF 50 stops new ones. Niacinamide, azelaic acid, alpha arbutin, vitamin C and tranexamic acid lighten existing ones over roughly twelve weeks. A picosecond laser clears defined sun spots faster, usually in two to four sessions. Anything promising a result in a week is not safe.
Does pekas come back?
Freckles do, with sun. Sun spots stay away far longer if you protect the skin, and return where you do not. Melasma returns unless it is managed, which is why it gets its own article.
Is laser treatment painful?
A brief snap against the skin. Clients at our Makati consultations put it at 2 to 3 out of 10, and rarely at 4. A larger area takes longer, so the session feels different from a few spots. Small areas need no anaesthetic and there is no time off work.
Can I treat pekas at home?
Home care can gradually improve superficial pigment, and sunscreen prevents new spots. Deeper or clearly defined spots are worth assessing before you treat them. What you should not do is any of the internet remedies involving lemon juice, calamansi or bleach on the face: an acid burn leaves pigment worse than the spot did.
Where can I have this done in Manila?
We assess pigment before quoting anything, because the plan for freckles and the plan for melasma are not the same. The clinic is in the lobby of The Peninsula Manila on Makati Avenue, with three other branches in Metro Manila.
