Four treatments get sold as a facelift alternative in Manila. They do different jobs and fail on different faces. A side by side comparison, including the two we do not perform.
Four treatments get sold as a facelift alternative in Manila. They do different jobs, they fail on different faces, and a clinic that offers only one will tell you yours is the one you need.
We run two of the four. The comparison below says where the other two win.
Start with the pinch, not the menu
Pinch the skin under your jaw and let go.
It springs back. Your skin has lost firmness and not yet lost its elasticity. Every non surgical option below is on the table.
It stays folded. That skin is surplus. Stimulating collagen does not remove surplus, and every treatment below will disappoint you. A surgeon removes it.
There is nothing to pinch and the face looks flat. That is volume loss, not laxity. Tightening a hollow face makes the hollow more obvious. Filler or fat transfer is the conversation.
Most disappointment in this category comes from treating the second and third groups with tools designed for the first.
Focused ultrasound, HIFU
What it does. Heats small points at 4.5mm, 3.0mm and 1.5mm. The 4.5mm layer is the SMAS, the sheet a surgeon lifts in a facelift. Deepest reach available without a cut.
Best on. Softening jawline, sagging cheeks, early jowls, mild neck laxity. Thirties to fifties.
Result. Visible at eight to twelve weeks, holds six to twelve months.
Downside. Nothing to see for the first month, which people read as failure. Numb patch along the jaw for up to four weeks. No effect on surface texture or pigment.
Pigment risk on morena skin. None. Ultrasound does not see melanin.
Radiofrequency microneedling
What it does. Needles carry current below the surface, so the heat lands in the dermis and the top layer is spared.
Best on. Texture, large pores, acne scars, crepey skin under the eyes, mild laxity. It is the texture tool of the four.
Result. Builds over three to four sessions a month apart.
Downside. Pinpoint bleeding on the day, redness for two or three days, and it lifts less than ultrasound. Real hyperpigmentation risk on Fitzpatrick four and five if settings run hot, so ask the operator how often they treat skin like yours.
Thread lift
What it does. Absorbable barbed sutures are passed under the skin and anchored, pulling tissue upward mechanically. The threads dissolve over months and leave collagen along the track.
Best on. A jowl or a cheek that needs to move now, before an event, on someone not ready for surgery.
Result. Immediate, which is the thing the other three cannot offer.
Downside. The most invasive of the non surgical three. Bruising and swelling for a week, dimpling or visible puckering while it settles, and a small risk of a thread migrating or becoming palpable. Lasts about a year. Costs more than HIFU in most Manila clinics.
Facelift surgery
What it does. A surgeon lifts the SMAS and removes the surplus skin.
Best on. Skin that hangs. The group the pinch test puts in category two.
Result. Years, not months.
Downside. General or twilight anaesthesia, two weeks of visible recovery, scars, and a cost that is a multiple of everything above. It is also the only option on this page that removes skin.

Combining them
HIFU and radiofrequency microneedling do different jobs and work well in the same year. Space them at least two weeks apart and treat them as separate courses rather than stacking them in one week.
Threads and HIFU in the same session is a sales package, not a protocol. Ultrasound heat near freshly placed threads is not something any clinic should be confident about.
What we perform
ICONIQUE runs HIFU and radiofrequency treatments at The Peninsula Manila in Makati, at SM Mall of Asia in Pasay, at Greenhills in San Juan and at Ayala Malls The 30th in Pasig. We do not place threads and we do not operate. When the pinch test puts you in the surgical group, the consultation says so and you keep your money.
Frequently Asked Questions
What is the best non surgical face lifting treatment?
For lift, focused ultrasound reaches deepest. For texture, radiofrequency microneedling. For an immediate change, threads. Which is best depends on whether your problem is laxity, surface quality or timing, which the pinch test sorts in ten seconds.
Is HIFU better than a thread lift?
They solve different problems. Threads move tissue on the day and last about a year. HIFU stimulates your own collagen, shows at eight to twelve weeks and carries no risk of a visible thread. Threads cost more in most Manila clinics.
Can a non surgical treatment replace a facelift?
Not when skin hangs. Every treatment on this page tightens or stimulates the skin you have. Only surgery removes skin you no longer need. Anyone promising a facelift result without a cut on loose skin is selling a word.
How much do these cost in Metro Manila?
HIFU runs from promotional rates in the low thousands to tens of thousands depending on line count and area. Threads usually cost more than HIFU per session. Surgery is a multiple of both. Ask every clinic for the count: lines for ultrasound, number of threads for a lift.
Which is safest for morena skin?
Focused ultrasound, because it does not interact with melanin at all. Radiofrequency microneedling is safe in trained hands and is the one on this list with a real pigment risk when settings are copied from lighter skin.
How old is too young for these?
There is no age gate. There is a skin gate. If nothing is loose yet, none of these gives you a visible result and the money is better spent on sunscreen, which is the only intervention that prevents the laxity you are trying to treat.
