An underarm diamond peel buffs the dead top layer off your underarm skin with a diamond-coated tip while suction lifts the debris away. It smooths rough skin and clears the grey film that dead cells build up in a fold that sweats and rubs, and you can feel the difference after one session. It does not reach the brown pigment behind most dark underarms, because that pigment sits in the living layers underneath. Before you pay for a course, work out where your colour sits: friction, razors, deodorant and hormones each leave it at a different depth.
Clinics abroad call it underarm microdermabrasion. It runs on the same machine as a facial diamond peel, with the same limits.
The layer a diamond tip removes
Your dead outer layer, the stratum corneum, is 10 to 15 micrometres thick, less than a sixth of a sheet of printer paper. A diamond tip thins it or takes it off. In a lab study on pig skin, short exposures removed the dead layer and left the living skin intact, while holding the handpiece on one spot for 20 seconds took living skin off as well.
Melanin starts further down. Melanocytes, the cells that make it, sit in the basal layer at the bottom of the epidermis and pass pigment to the skin cells around them. Those cells carry it upward as they mature, and they still hold some of it when they die and reach the surface. A peel removes that surface share. The melanocytes below keep working.
A course can lower epidermal melanin over time. In two biopsy studies, one on forearms and one on facial melasma, melanin dropped after a series of sessions, and the melasma study rated the visible change mild to moderate. No published study has tested microdermabrasion on underarms: PubMed returns nothing for the two terms, and a 2025 review of treatments for dark underarms found ten studies, none on microdermabrasion. Treat any session count a clinic quotes for an underarm diamond peel as that clinic's own experience.
Depth depends on the cause
A 2006 study took biopsies from the underarms of 20 Filipino women. In the women with dark underarms, the researchers classed the extra colour as mild post-inflammatory hyperpigmentation: melanocytes stimulated by irritation, putting more melanin into the epidermis. Hair plucking stood out as a trigger. Around plucked follicles, a few more immune cells had swallowed pigment leaking from the damaged epidermis, leaving some colour in the dermis.
Friction. Tight sleeves, bra bands, bag straps, and skin rubbing on skin in humid heat. Friction pigment sits in the basal layer, and long rubbing also thickens the dead layer above it, the way a callus builds where a shoe rubs. A diamond peel takes off the thickened surface, so friction darkening looks lighter and feels smoother after the first session. The basal pigment stays while the rubbing continues.
Shaving and plucking. In a 2020 study of Thai women, one hair-removal session raised redness and dryness and left no new pigment 48 hours later. Shaving twice a week for years keeps the skin inflamed between razors. Two marks of hair removal sit beyond a diamond tip: stubble showing through thin skin as a blue-grey shadow, and pigment in dots around plucked follicles, part of it in the dermis. Laser hair removal clears the shadow, and putting down the tweezers stops new dots.
Deodorant. Alcohol and fragrance irritate thin skin, and mild irritation adds pigment to the epidermis the same way friction does. A true allergy to a fragrance ingredient can go deeper. In pigmented contact dermatitis, which dermatologists have traced to fragrance chemicals such as benzyl salicylate, the reaction damages the basal layer and melanin drops into the dermis, where it looks grey-brown. No peel reaches it. A dermatologist can patch test you to find the ingredient.
Hormones. Two different things share this word. More than 90 percent of pregnant women develop some darkening, and the underarms are one of the usual sites. It fades over the months after delivery, though it may not fade all the way, so wait before paying for treatment. Insulin resistance, common with PCOS and weight gain, can cause acanthosis nigricans: dark, thick, velvety skin, often on the back of the neck and in the groin too. The colour comes from thickened skin more than from melanin. A peel thins it for a while, the thickening returns while insulin stays high, and it improves when a doctor treats the cause.
You can read a clue in the colour before any biopsy. Brown that stays inside the area a sleeve or strap touches points to friction and epidermal pigment. A grey, slate or purple-brown tint points to pigment in the dermis, which creams, peels and lasers struggle to clear, so show it to a dermatologist. Velvety thickening points to acanthosis nigricans.
Benefits of an underarm diamond peel
- Smoother skin after one session, because the rough, thickened surface comes off.
- Less of the grey, ashy cast that dead cells give a fold where sweat and deodorant sit.
- Better absorption for a week or two. A brightening product reaches living skin instead of dead cells, so a peel works well as the step before one.
- A lower risk of dark marks than a chemical peel on Filipino skin, since no acid and no heat touch it.
- Fast barrier recovery. In a Korean study of 28 patients, water loss through facial skin returned to baseline on the second day after a diamond tip.
A diamond peel is abrasion, and abrasion is friction. Sessions about four weeks apart put less of it on your skin than a razor twice a week. The same tip over a fresh razor scrape, an active ingrown hair or a boil adds the irritation that makes pigment, so settle those first.
Sessions, and when to judge them
With no underarm trial to go on, use the evidence nearby. Published microdermabrasion courses on the face run four to eight sessions. Laser trials on underarm pigment saw change by the third session, and in one of them more sessions did not predict a better result.
A workable plan: book three sessions about four weeks apart, the time skin takes to renew, and judge the colour four weeks after the third, twelve weeks from the start. Photograph your underarm before the first session, arm raised, facing a window in daylight, and repeat the same shot before each session and at the twelve-week check. If the texture has improved and the brown has not moved by then, the rest of the colour sits below the tip. More peels will keep the surface smooth without reaching that pigment.
The case for a laser
Switch to a laser when the colour survives three peels and a month without the cause: a trimmer instead of a razor, no tweezers, looser sleeves, a deodorant without fragrance. Colour that survives both sits below the dead layer, where laser light reaches it: the light passes through the surface, and melanin absorbs it where it sits.
Most of the underarm evidence comes from Q-switched lasers at 1064 nm. In a 2020 study of nine Filipino women with skin types IV to V, four sessions two weeks apart reduced pigment across 85 to 100 percent of the dark area in all 18 underarms. The change held at the three-month check, and the study recorded no side effects. A 2021 Thai trial of 22 women found significant improvement after three sessions.
Picosecond lasers fire pulses measured in trillionths of a second, against billionths for Q-switched lasers. Across 21 comparison studies and 971 patients, picosecond lasers cleared pigment as well, hurt less and left fewer dark or light marks, with the safety edge clearest in Asian patients. No underarm trial has used one yet.
