A bloated face in the morning and heavy, swollen legs by evening come from the same thing: fluid that has leaked out of your small blood vessels and collected in the tissue faster than your body carries it back. Gravity decides where it collects. Lying flat all night moves it to your face; standing or sitting all day sends it to your ankles. Salty food, heat and the days around your period add to it. This kind of swelling goes down within a day once the trigger passes. Swelling in one leg, swelling with breathlessness, or a puffy face with foamy urine is a different matter, and the last section tells you when to see a doctor.
Why fluid collects in the first place
Your blood vessels leak a little plasma into the surrounding tissue all day. Most of it seeps back into the veins. The rest drains through lymph vessels, a second network of thin tubes that returns it to the bloodstream. Lymph vessels have no heart to push them. Stay still for hours, or eat something that makes your body hold extra water, and fluid builds up where gravity pulls it. Press a thumb into a swollen ankle for a few seconds and the dent that stays shows you the fluid.
A bloated face in the morning
You spend seven or eight hours lying flat. Gravity stops pulling fluid down to your feet, and it spreads through your whole body, face included. The skin around your eyes is the thinnest on your face, so puffiness shows there first. Once you are up and moving, gravity pulls the fluid back toward your legs.
Salt makes it worse. Your kidneys hold on to water to keep the sodium in your blood at a steady concentration, so a salty dinner means more fluid on board by morning. The WHO advises adults to keep salt under 5 grams a day, a little under one teaspoon. The global average in 2021 was 11 grams. Instant noodles, bagoong, patis, toyo and chicharon all count.

Heavy, swollen legs by evening
A day behind a counter or at a desk keeps your calf muscles idle, and fluid pools in the lower legs. In a study of 10 men standing for four hours, the calf grew 1.32% in circumference when they stood still. Moving their ankles for two minutes, twice an hour, cut that to 0.61%. Sock lines that leave a deep mark and shoes that feel tight by 5 pm are the signs you will notice.
Heat adds to it. Your blood vessels widen to shed heat, more fluid leaks out, and doctors call the result heat edema, the mildest form of heat-related illness. A long commute in Manila traffic on a hot afternoon combines both triggers: you sit still, and you are warm.
The NHS advice for swollen legs is to raise them on a chair or pillows when you can, walk to get the blood moving, and avoid standing or sitting in one position for long.
Water retention before your period
Many women feel bloated in the days before their period. The timing differs from what most articles say. In a year-long study of 62 women across 765 menstrual cycles, the women scored their fluid retention each day on a scale of 0 to 4. The average score peaked on the first day of bleeding, at 0.9, and fell to its lowest in the days after the period ended. It also crept up from 0.22 to 0.50 over the 11 days around ovulation. Blood levels of estrogen and progesterone did not predict who retained more.
Two points follow for you. Expect the bloating to peak on day one of your period and ease over the following week. And an average under 1 out of 4 on the worst day means the women in the study rated their swelling as mild.

Swelling that needs a doctor
Water retention from salt, heat, sitting or your cycle affects both sides of the body and goes down within a day or so. See a doctor first, and skip pressotherapy, in these cases:
- One leg swollen, painful, red or warm. A blood clot in a deep vein can cause this. The NHS treats it as urgent. Pressotherapy waits until a doctor has ruled out a clot.
- Swelling with shortness of breath, chest pain or coughing blood. Go to the emergency room. A clot may have reached the lungs.
- A puffy face and eyes with foamy urine. MedlinePlus lists both among the symptoms of nephrotic syndrome, a kidney disorder.
- A puffy face, hands and feet with tiredness, feeling cold and weight gain. These belong to an underactive thyroid, which a blood test confirms.
- Face or lips swelling fast, or trouble breathing. An allergic reaction can close the airway. This is an emergency.
- Swelling in both legs that has not improved after several days, or keeps getting worse. The heart, kidneys, liver and some medicines can cause it. Keep taking your medicines and ask the doctor who prescribed them.
